Welcome to Cambridge Admissions. Please fill out this online form if you are a new family enrolling in our school or if you would like to attend our summer camps.  All new families who are seeking to enroll at Cambridge must have toured the school prior to completing this form (campers do not have this requirement) and have been given notice that there is sufficient room for your child to enroll in our school.. Upon receipt of this registration form you will be contacted by the administration to finish the enrollment process and schedule your parent orientation. Campers are not required to attend orientation.

    Informed Consent
    I wish to enroll my child in Cambridge International School and accept the following conditions:,
     
    Family Responsibilities
    All children participate in both indoor and outdoor activities daily. It is the parent's responsibility to not only dress the child appropriately but to apply Sun Block, Bug Spray and to provide a change of clothes and any other necessary items for their comfort. 
     
    Financial and Withdrawal
    -Tuition is due on or before the 1st of each month. On or after the 3rd of the month a $15 late fee will be assessed. If payment is over 20 days late, the parent will be asked to attend a conference. If account remains delinquent for over 30 days, your child/children may be removed from the roster until all sums owed have been paid.
    -Payments can be made by cash, check, credit card, or bank draft (Echeck). Payments by credit card will incur an additional 3% processing fee. Payments by other methods do not incur additional fees.
    -Parents wishing to withdraw a child from Cambridge must provide written notice within 60 days prior and continue to pay the full tuition for the month of the withdrawal . Failure to give 60 days written notice, parents are obligated to pay the full month, and in addition, the following month as well. If the child is withdrawn prior to the end of the school year, no deposits will be refunded for any reason.
    -Cambridge does not give refunds or credits for absences due to illness, vacation or any other reason.
    -Parents must fill out a medication form if you wish that we dispense medication to your child.  Parents must provide written authorization which includes: date, name of child, name of medication, dosage, and date and time of day medication is to be given. Medicine must be in the original container with child's name clearly marked.
    -In cases of illness or injury, we will immediately contact parents or emergency contacts. Minor injuries will be attended to by the Cambridge Staff with parents notified immediately via incident report and phone call., 
    -Staff may apply sunscreen or bug spray to students - please return the permission to apply topical medications, ointments etc. 
    -Parents must notify the school concerning allergies and proper protocol for addressing any allergy episode. Parents will provide medication forms and medication to administer at school should your child have a severe allergy that would  need immediate attention.
    -Cambridge will work to ensure that the school premises are properly maintained and will monitor the activities of the children in the context of the program, parents understand that accidents happen, and, therefore, releases and discharges and hold harmless Cambridgel and its agents, employees, and contractors from any and all claims, demands, damages, rights of action or causes of action, present or future, arising out of or connected with my child's participation in the program, including any injuries resulting therefrom, that do not directly result from the negligence of Cambridge's staff.
    -In case of medical emergency staff and administration will notify parents and emergency services. Staff will respond as necessary until EMS arrives. In the event emergency treatment is required, parents give consent for their children to be taken to a nearby medical facility and if necessary for treatment by a qualified physician. Costs incurred from treatment of any injury or illness occurring within the program is the responsibility of the parent.

    Privacy and Well Being
    -Parents are attesting that their children have no conditions which restrict their full participation in the Cambridge School program. 
    -Parents are responsible for keeping their contact updated and any new information relayed in a timely manner.
    -Photographs of students will be taken with the expressed permission of the Campus Director and may appear in websites, advertisements, publicity, media reports, and art projects by the students in the program. Should you elect to not have your child's photograph used, please let us know.
    -Cambridge Staff and Administration are mandated reporters and by law must call authorities if it is suspected that a child is being abused or is any danger. Signs of neglect will be reported and state officials have the right to inspect my child's file and interview him/her at any time without notification. 
    -Cambridge is a Quality Rated, fully licensed program. 
     
    Consent to Medical Treatment
    I hereby give permission for my child to be given emergency treatment, to include first aid and CPR by a qualified Cambridge staff member. I further authorize and consent to medical, surgical, and hospital care, treatment and procedures to be performed by a licensed physician or hospital when deemed necessary or advisable by the physician to safeguard my child's health should I be unable to be contacted.
